Ready to Serve Safely Enrollment Form

Completing this form enrolls your restaurant in the Ready to Serve Safely program and commits your restaurant to following the safety criteria outlined in the program.  Your restaurant will be listed on the Ready to Serve Safely website for customers looking for restaurants who are following COVID-19 best practices for health and safety within their restaurant. NOTE: Restaurants not adhering to the checklist of criteria will be removed from the program.

• Adhering to Wisconsin and local health department orders or mandates
